PERMIT#:M-1973-021 <br /> INSPECTOR'S INITIALS: ECS <br /> INSPECTION DATE:November 20,2018 <br /> OBSERVATIONS <br /> This was a "pre-operation" inspection for AM06 conducted by Eric Scott and Tim Cazier of DRMS. DRMS <br /> accompanied Aggregate Industries (Al) representative Nick Martin and their consultant for AM06,Jeremy <br /> Deuto of GEI, on a tour of the site after meeting at approximately 9:30 at the site office. The first part of the <br /> inspection was spent with Al and GEI at the site office discussing how best to address some of the issues that <br /> were identified by DRMS in the initial AM06 adequacy review letter. <br /> The site tour started at the office overlooking the North Reservoir and process areas (photos 2 and 3). The <br /> field conditions largely matched the information provided in AM06, however, more details regarding the <br /> existing structures on site (fuels station in central quarry, Town of Morrison water infrastructure, below grade <br /> drainage structures, storm water diversions, employee facilities, etc.) will need to be included in AM06 where <br /> appropriate. <br /> The areas of the North Quarry that have been reclaimed appear to be stable and well vegetated (photo 4). A <br /> small amount of knapweed was observed in some of the more recently disturbed areas and near the site <br /> office trailer. Weed control efforts will need to be maintained across the site where appropriate to keep <br /> noxious weeds from becoming established. <br /> The Central Quarry area has been fully backfilled to final grade at this time and is serving as a product <br /> stockpile and fueling area (photo 5). Although the fuel tanks were observed to be within a secondary <br /> containment structure, there were multiple other containers located within the same containment structure. <br /> The remaining containment volume should be evaluated to determine if sufficient secondary containment <br /> volume still remains. Some containers may need to be removed and placed in another appropriate location if <br /> they are taking up too much of the containment volume within the existing enclosure. <br /> The western high wall of the South Quarry (photo 6) is the currently active phase of the mine, however, no <br /> hauling or blasting was occurring during the inspection. This southern area of the permit is the focus of AM06 <br /> as the submitted amendment proposes steeper slopes than currently approved for the bottom portion of the <br /> south quarry to maximize resource extraction. Prior to and during the inspection, GEI was conducting the field <br /> work necessary to produce the geotechnical stability evaluation that is a required part of the annual report for <br /> this site. Slope survey/monitoring points were observed in several locations in the south quarry so that the <br /> operator can monitor for any slope movement and evaluate if corrective action is necessary. During the <br /> inspection the slopes and benches appeared to be largely stable, and the completed upper portions of the <br /> active slope are currently under reclamation and revegetation. <br /> The only issue requiring correction noted during the inspection is that the proposed permit boundary/affected <br /> area will need to be fully marked according to rule 3.1.12(2) as noted on the first page of this report. The <br /> operator will need to provide proof of marker placements (such as dated photos) and a surveyed map showing <br /> the complete proposed permit boundary for AM06 by the corrective action due date listed (12/20/18). <br /> Page 2 of 4 <br />
