Everist, Incorporated (L.G. Everist or LGE), as Permittee/Operator respectfully submits this Corrective Action <br /> Report for the Firestone Gravel Resource, DRMS Permit# M-1996-052. This Report is pertaining to the possible <br /> problem noted on process water in Firestone's Inspection Report dated 10/22/2018. The due date for the process <br /> water Corrective Action is 11/22/2018. <br /> On the day of the inspection, it was noted in the report that"process water/sediment was observed leaving the <br /> site along the western permit boundary near the plant area..." <br /> The water which was seen in the road-side ditch on the day of the inspection was water that had been pumped <br /> from the settling pond in the southwest corner of the site, and was being pumped to the fresh water pond in the <br /> north-central part of the site. There had been a minor back-up in the on-site ditches between the two ponds, so <br /> some of the water filtered over to the road-side ditch. This was noticed almost immediately and the water transfer <br /> was shut off. The day of the inspection, was the day after this occurred, and there was some water still sitting in <br /> the road-side ditch. The remaining water was cleaned up later that day and then the ditch was bladed smooth. <br /> The water was only in the road-side ditch for about 100 feet in the middle of the western perimeter and that ditch <br /> dead-ends at the northwest corner of our site, so the water did not have any chance of traveling to other areas. <br /> Please see the attached photos showing the ditch after the water was pumped back into the site and the ditch <br /> was cleaned up, graded, and smoothed out. <br /> If you have any questions or need additional information pertaining to the process water cleanup, feel free to <br /> contact me at 303-286-2247 or Imshults@lgeverist.com.
