<br />actively mining. The operator may follow a reduced monitoring plan (Exhibit 29, Appendix D)
<br />that the Division approved for implementation while the mine is in temporary cessation.

<br />Ground Water Monitoring Plan
<br />
<br />The Water Quality Monitoring schedule for the Williams Fork Mines is found in Exhibit 29 of
<br />the permit application. There are ten bedrock ground water monitoring wells and six alluvial
<br />monitoring wells. Six of the ten bedrock monitoring wells-the No. 5 Mine, TR-4, TR-7A, 81-01,
<br />259 and the No. 9 Mine Well are sampled quarterly and annually for the parameters listed shown
<br />below.

<br />Quarterly: Temperature, Conductivity, pH and Water Level
<br />
<br />Annually: Temperature, Conductivity, pH, Water Level, Alkalinity, Hardness, Calcium,
<br />Magnesium, Sodium, Bicarbonate, Carbonate, Chloride, Sulfate, Arsenic, Cadmium, Lead,
<br />Mercury, Selenium, Iron, Manganese, Zinc, Boron, Molybdenum and Nitrate.

<br />Water levels in the four remaining bedrock monitoring wells-83-01, 83-02, 83-03, and 84-01-are
<br />measured quarterly.
<br />
<br />Alluvial valley wells AVF-3, AVF-5, and AVF-6 are also monitored for the quarterly and annual
<br />parameters listed above for the bedrock monitoring wells.
<br />
<br />Alluvial Wells YAW-1, YAW-2, and YAW-5 or YAW-6 will be monitored quarterly and
<br />annually for the parameters listed below in conjunction with the development of the North Mains
<br />if and when this occurs in the future:

<br />Quarterly: Temperature, Conductivity, pH and Water Level
<br />
<br />Annually: Temperature, Conductivity, pH, Water Level, Alkalinity, Hardness, Calcium,
<br />Magnesium, Sodium, Bicarbonate, Carbonate, Chloride, Sulfate, Arsenic, Cadmium, Lead,
<br />Mercury, Selenium, Iron, Manganese, Zinc, Boron, Molybdenum, Nitrate, Ammonia, Nitrite and
<br />Phosphate.
<br />
<br />While the Williams Fork Mines are in temporary cessation status, the water monitoring schedule
<br />found in Appendix D of Exhibit 29 will be in effect. In accordance with the approved plan, Well
<br />TR-7A will be monitored annually for water level and field parameters for the well discussed
<br />above. The No. 5 Mine Sump will be monitored quarterly for the water level of the sumped
<br />water at the well bore location measurements.

<br />Surface Water Monitoring Plan
<br />
<br />Water quality in the Williams Fork River is monitored at an upstream site and a downstream site.
