§2.04 APPLICATION FOR PERMIT FOR SURFACE OR UNDERGROUND MINING <br />ACTIVITIES --MINIMUM REQUIREMENTS FOR INFORMATION ON <br />ENVIRONMENTAL RESOURCES. <br />2.04.13 Annual Reclamation Report. <br />(1) By February 15th of each year the applicant will file an annual <br />reclamation report covering the previous calendar year for all areas <br />disturbed under bond. The report will include text, discussion and <br />maps which address: <br />(1)(a) The name and address of the permittee and permit number; <br />(1)(b) Location and number of acres disturbed during that year; <br />(1)(c) Location and number of acres backfilled and graded during that year; <br />(1)(d) Location and number of acres topsoiled during that year; <br />(1)(e) The species, location and number of acres of vegetation planted during that <br />year, including any augmented seeding or cultural practices; and <br />(1) <br />i~ <br />(1)(f) Location, number of acres and date of planting for all previously revegetated <br />areas. <br />No response required. <br />TR-20 <br />2.04 - 35 <br />3/01 <br />
