PERMIT#: M-1996-052 <br /> INSPECTOR'S INITIALS: ECS <br /> INSPECTION DATE:October 22,2018 <br /> OBSERVATIONS <br /> This was an acreage release inspection for an active 112c sand and gravel operation conducted by Eric Scott of <br /> DRMS for the Firestone Gravel Resource site operated by LG Everist. DRMS met with permittee <br /> representatives led by Lynn Mayer Shults at the site at approximately 11:00 and toured the site with them. The <br /> release request for 27.98 acres was submitted to DRMS on October 10, 2018, and DRMS sent out the required <br /> notices on October 12, 2018. The public comment period will expire on October 29, 2018 and to date no <br /> objections have been received to the proposed partial release. <br /> The portion of the site to be released consists of 27.98 acres located along the south, southeast corner, east, and <br /> northeast sides of the permitted area as depicted on the maps submitted with AROL This area has not been <br /> affected by mining activity and remains unaffected rangeland (see attached photo). <br /> The Town of Firestone is acquiring this unaffected property through a land exchange with the permittee for <br /> their own purposes (open space and trails). Because the 27.98 acres have not been impacted by mining activity, <br /> DRMS will support the release of this area from the permit at this time and AROI will be finalized when the <br /> public comment period expires. <br /> During the inspection of the Firestone site, several items were noted that will require revisions to the current <br /> mining plan through either Technical Revision, or possibly a permit amendment, by the corrective action date of <br /> January 20, 2019 as stated on the first page of this report. These items are summarized below. <br /> 1) The maximum allowable disturbed acreage for the Firestone site is 95 acres, however, based on review <br /> of aerial photography from May 2018 it appears that approximately 150 acres have been disturbed by <br /> mining activity (see attached map). <br /> 2) The unlined excavation to be utilized as a sediment settling pond has been relocated from its previous <br /> location in the SW corner of the permit. <br /> In addition, process water/sediment was observed leaving the site along the western permit boundary near the <br /> plant area—this issue will need to be resolved, cleaned up, and proof of the correction submitted to DRMS <br /> within 30 days. <br /> Inspection Contact Address <br /> Lynn Mayer Shults <br /> L.G.
