Black Canyon), Permit No. M-1977-210; Corrective Action Field Work <br /> Completion. <br /> Dear Mr. Cazier: <br /> Transit Mix Concrete Co., on behalf of Continental Materials Corporation, is informing the Division <br /> that the corrective actions required at the Snyder Quarry were completed on or about October 2, <br /> 2018. With the completion of the reclamation of the Water Supply pond, and regrading the upper <br /> access road, we believe we have completed the following, required corrective actions. <br /> INSPECTION TOPIC: Roads <br /> PROBLEM/POSSIBLE VIOLATION: Problem No. 1:The road between Phases WP and AP is in especially poor <br /> condition requiring a four-wheel drive vehicle and careful navigation of the road. <br /> CORRECTIVE ACTIONS: Provide photographic evidence of road repair and maintenance to remove/repair the <br /> erosion rilling in the road designated as a fire escapeway by the Corrective Action Due Date. <br /> CORRECTIVE ACTION DUE DATE: 5/21/18 <br /> This work was completed and signed off by Mr. Cazier on June 18, 2018. <br /> INSPECTION TOPIC: Reclamation Success <br /> PROBLEM/POSSIBLE VIOLATION: Problem No. 2:The water storage pond added for the rock wash plant as part <br /> of amendment 2 (AM-02) is in poor condition due to lack of maintenance,with the liner torn and ineffective. <br /> CORRECTIVE ACTIONS: Reclaim the water storage pond as approved by the Division in Amendment 4. Provide <br /> photographic evidence of removing the liner prior to any backfill placement,followed by photographs of <br /> achieving final grades,and seed tags for the revegetation effort or if appropriate, a schedule for later seeding in <br /> 2018 by the corrective action due date. <br /> CORRECTIVE ACTION DUE DATE: 6/19/18 <br /> For problem number 2 cited in the Inspection Report, please note the attached photographs, seed <br /> tags, and reclamation contractor invoice document the corrective actions taken at the water supply <br /> pond. In addition, some erosion control mats were added to help with slope stabilization during seed <br /> establishment. <br /> In the original seed mix submitted to the DRMS as part of the corrective actions, mountain <br /> mahogany was not listed. We included mountain mahogany since it is in the site approved seed mix. <br /> Also, it is an abundant shrub surrounding the site and within the basin side slopes of the water supply <br /> pond. <br />
