The 13H-IR intersected <br /> the current 13H mining interval from the west. NS retains the ability to inject from the existing <br /> 13H-1 well and use the 13H-IR as either an injection or recovery well as sodium bicarbonate <br /> (baking soda) production dictates. The preexisting NS DS-3 ground water monitor well will be <br /> utilized as the subsurface subsidence monitor well (SSMW) for the 13H-IR well. The DS-3 <br /> penetrates the R-6 confining interval and is proximal to the 13H-IR well bore path. DS-3 well <br /> completion data was previously submitted to the EPA during the 12H-IR permitting process. <br /> The existing 13H-SSM (surface subsidence monument) is known to the EPA, located above the <br /> 13H mining interval, and is surveyed per the NS UIC permit. <br /> Please find attached: <br /> • Aerial and topographic maps indicating proposed SSMW location and the '/4 mile the Area of <br /> Review(AOR) <br /> • Completed 7520-6 form <br /> • Completed 7520-14 form <br /> • Completed 7520-9 form <br /> • Comprehensive list of the AOR wells <br /> • As-Built Well Completion Diagrams for the Injection/Recovery Well <br /> • Well Plug and Abandonment Diagram <br /> • Well Completion Data <br /> • Stratigraphy and Hydrostratigraphy Tops <br /> • Capstar Daily Drilling Reports <br /> • Open Hole Geophysical Logs <br /> • Halliburton Cementing Job Report, Jet West CBL, and Cement Evaluation <br /> • Scientific Drilling End of Well Report(directional surveys) <br /> • MIT Part I Report and Data <br /> • Equipment Layout and Skid Specifications <br /> The maximum allowable injection rate (MAIP) for NS injection wells is 300 psi. Per the NS UIC <br /> permit there are no specific limitations on injection well flow rates or volumes. <br /> Well completion information and diagrams for all existing wells in the AOR remain unchanged <br /> and have been previously submitted to the EPA and approved on multiple occasions.
