Three copies of this permit application have been submitted to CDRMS at 1313 Sherman Street,
Room 215, Denver, CO. Copies have also been provided to the OSMRE at 1999 Broadway,
Suite 3320, Denver, CO, and the Bureau of Land Management (BLM) Little Snake Field Office
at 455 Emerson Street, Craig, CO.
2.03.3(9) Certifcation by Responsible Offical
The information contained in this permit application is true and correct to the best of current
knowledge, information and belief. The responsible offical has certified this permit application
in accordance in the rules in the cover letter application.
2.03.4 Identification of Interests
Colowyo Coal Company L.P. is a limited partnership between two wholly owned subsidiaries of
Elk Ridge Mining and Reclamation, LLC. It's General Partner and Manager is Axial Basin Coal
Company, which hold a 20% partnership interest in the Colowyo Mine. The remaining 80%
partnership interest is held by Taylor Creek Holding Company. Elk Ridge Mining and
Reclamation, LLC is wholly owned Tri-State Generation and Transmission Association, Inc.
Elk Ridge Mining and Reclamation, LLC, is the permit applicant and mine operator for the
Colowyo Mine (Colowyo). The general office is located at 5731 State Highway 13, Meeker,
Colorado 81641; telephone number [phone]. Colowyo's Employer Identification
Number is 87-0522903. Colowyo's Mine Safety and Health Adminstration (MSHA)
identification number is 05-02962 and it was issued on October 1, 1976.
The duly Authorized Representative for Elk Ridge Mining and Reclamation, LLC is Barbara
Walz, Chief Compliance Officer for Tri-State Generation and Transmission Association, Inc.
The resident agent for Elk Ridge Mining and Reclamation, LLC is Ken Reif, Senior Vice

Colowyo Coal Company L.P. will remit all required AML fees.
