Also please include available information on the existing well located in the far SW corner <br /> of the site(near MW5) and include it on the existing conditions map(sheet 2)provided. <br /> EXHIBIT E-Reclamation Plan (Rule 6.4.5): <br /> Seeding-specify types, mixtures,quantities,and expected time(s) of seeding and planting.The reclamation <br /> seed mix and plans for revegetation have not been provided. Please address. <br /> Please remove the reference to Bestway Concrete in the last sentence of the weed control plan. <br /> The post mining land use for the majority of this permit is developed water storage(lined reservoirs). Because the <br /> proposed reservoirs are all or partly within the Platte River floodway,the permittee will be required to provide an <br /> engineered design for spillway(inlet/outlet)structures to be incorporated into the reclamation plan to protect the <br /> integrity of the reservoirs during flood events. The flood control structures will also need to be shown on the <br /> Reclamation Plan Map. <br /> EXHIBIT G-Water Information(Rule 6.4.7): <br /> Please commit to providing DRMS with at least 5 quarters(15 months)of groundwater level data from the 5 <br /> existing monitoring wells to determine baseline maximum and minimum pre-mining water table elevations before <br /> initiating work on-site that will impact groundwater levels in the area(installation of slurry walls,dewatering,etc). <br /> This baseline data will evaluated in the event that groundwater level mitigation is required. <br /> The groundwater monitoring and mitigation plan as submitted states that if trigger levels within the existing wells <br /> are exceeded(excess of 2ft deviation from historic background levels),or if a complaint of off-site impact is <br /> received,the permittee will take action and notify DRMS within 30 days. <br /> In the event that trigger levels are exceeded,or complaints of off-site groundwater impacts are received,please <br /> commit to notifying DRMS within 5 working days. The provided notice shall include, at a minimum, a description <br /> of the issue observed or reported(excessive mounding or shadowing, off site impact, etc), additional data to be <br /> gathered by the permittee to characterize the issue if needed, and possible corrective actions that may be <br /> implemented to resolve the issue if deemed necessary. <br /> EXHIBIT L-Reclamation Costs (Rule 6.4.12): <br /> All information necessary to calculate the costs of reclamation must be submitted and broken down into the various <br /> major phases of reclamation. You must provide sufficient information to calculate the cost of reclamation that <br /> would be incurred by the state. <br /> DRMS will further evaluate the provided reclamation costs when all other adequacy issues have been resolved. At <br /> this time it appears that,at a minimum,additional information will be required regarding costs to backfill mined <br /> slopes from 0.5H:1 V within all excavated areas,backfill and fully reclaim the sediment pond and clean water ponds, <br /> install engineered inlet/outlet structures for all remaining open water reservoirs,and provide documentation to <br /> substantiate unit costs for slurry wall installation. <br /> EXHIBIT S-Permanent Man-Made Structures(Rule 6.4.19): <br /> Please note that roadways and above-ground or underground utilities(if present)within 200 feet of the proposed <br /> affected area are considered permanent man-made structures. In accordance with Rule 6.4.19,when mining <br /> operations will adversely affect the stability of any significant,valuable and permanent man-made structure located <br /> within 200 feet of the affected area,the applicant may either: <br /> (a) Provide a notarized agreement between the applicant and the person(s)having an interest in the structure, <br />
