PERMIT#: M-2018-037 <br /> INSPECTOR'S INITIALS: ECS <br /> INSPECTION DATE: September 28,2018 <br /> OBSERVATIONS <br /> This was a pre-operation inspection for the proposed 112c Inouye Gravel Mine. The site was inspected by DRMS <br /> representative Eric Scott who was accompanied by permittee representative Mark Johnson, and permitting consultant <br /> Andy Rodriguez. The proposed 122 acre permit area is accessed from the south side of Weld County Road(WCR) 14.5 <br /> approximately 0.7 miles west of the intersection of WCR 14.5 and Hwy 85. The site is was previously utilized for <br /> rangeland and dryland hay and has a vacant residence and outbuildings located near the center of the eastern portion of the <br /> permit area. <br /> As shown in the attached photos, the permit applied for notice was clearly posted in two locations adjacent to WCR 14.5, <br /> and the proposed permit boundary was marked with steel posts with pink flagging tape. All structures observed during <br /> the inspection were as depected on the provided. The only structure noted as missing from the maps is an old(possibly <br /> abandoned/inoperable)well located in the far SW corner of the proposed permit near MW-5. This feature should be noted <br /> and added to any subsequent maps, and any available information on the construction and ownership of the well should be <br /> provided in Exhibit G—especially if the well will be utilized by the proposed operation. <br /> No other issues were noted during the inspection, and no objections have been received by DRMS during the public <br /> comment period. Copies of comments letters from other entities(USACOE, DWR, and CPW)were provided to Mr. <br /> Rodriguez for the permittee's file.
