(OSMI)received the preliminary adequacy review(PAR)regarding the <br /> application for Technical Revision 10 to Permit M2012-032 for the Revenue-Virginius Mine(also known <br /> as the Revenue Mine). DRMS raised 4 concerns, each of which is addressed below under headings that <br /> repeat the DRMS issue in italics. <br /> 1) Regarding Pond#2 in Section 4.1 Five Stage Passive Treatment System, Page 3, states that <br /> all existing media may potentially be placed in the tailings facility.Is there any elevated <br /> metal concentrations that have potential to leach out and be a source of contamination? <br /> While OSMI anticipates that bioreactive materials from the existing passive treatment <br /> system will be reused,and that metals leaching would be low, OSMI would perform <br /> Synthetic Precipitation Leachate Procedure(EPA1312)testing on the materials before <br /> placement in the tailings facility. If leachate quality indicates off-site disposal is <br /> necessary,the materials will be taken to an appropriate off-site facility and the necessary <br /> additional testing(i.e.TCLP EPA 1311) will be performed to determine the appropriate <br /> facility. <br /> 2) Section 7.2 Groundwater Monitoring Well Installation states an abandonment report will be <br /> filed with the State Engineer. Please commit to also submitting abandonment reports for all <br /> groundwater monitoring wells to DRMS. <br /> OSMI will supply abandonment reports for any abandoned groundwater monitoring wells <br /> to DRMS as well as the State Engineer's Office. <br /> 3) Maps and figures must be signed and dated by a qualified engineer and map maker. All <br /> figures and maps submitted with the Technical Revision request were not signed or dated. <br /> Please submit maps and figures with signatures and dates. <br /> Please find all maps and figures attached with signatures and dates. <br /> 4) Section 4.2 Groundwater Monitoring Well Installation states the approximate location of <br /> GW-4 can be seen on Map I and Figure 2.
