31 August 2018 <br /> <br />1 <br /> <br />Oldcastle SW Group, Inc. dba Four Corners Materials (FCM) <br />SOUTH FORK #1 PIT – Mine ID# 2017‐004 <br />RE: Response to Inspection Report dated August 2, 2018 by Stephanie Mitchell <br /> <br /> <br />Corrective Action: Utilization of off‐site fill <br />As was discussed during the field inspection on July 27, 2018, the material stockpiles present on the site <br />originated in or at the adjacent O’Bannon Pit. These materials, un‐processed recyclable asphalt and 3/8” <br />chips, went unused during a FCM highway paving project completed in 2016. <br />The intent was to preserve these materials for future use as recycled asphalt product (RAP) and as chip <br />seal or in other asphalt mixes. The State Land Board (landowner) approved the temporary storage of <br />these materials as part of the mining and surface rental lease agreement FCM holds. <br />FCM has no intention of using either of these materials for backfill or any fill purposes on this or any <br />other mine site. <br /> <br /> <br /> <br /> <br />
