Subcontract#: 061804-017 <br /> Page 33/Rev 171204 <br /> (c) USL&H (if applicable) $ Statutory <br /> (d) Stop Gap Coverage $ 1,000,000 (Policy Limit) <br /> (e) Employers Liability $ 1,000,000(Each Accident) <br /> $ 1,000,000(Each Employee by Disease) <br /> $ 1,000,000 (Policy Aggregate for Disease) <br /> Subcontractor's Workers' Compensation coverage shall be in the form and with limits in accordance <br /> with the laws of the state of jurisdiction, including without limitation Occupational Disease Insurance, <br /> Voluntary Compensation Insurance and United States Longshoreman's and Harbor Workers Act <br /> Coverage. <br /> IV. Commercial Umbrella or Excess <br /> (a) $ 5,000,000 (Each Occurrence) <br /> (b) $ 5,000,000 (Aggregate) <br /> Subcontractor's Umbrella Excess Liability Insurance Policy shall be at least as broad as the <br /> underlying Commercial General Liability, Automobile Liability and Employers Liability policies. <br /> V. Professional Liability <br /> If services are provided which are insurable under a professional liability policy, the minimum limits <br /> shall be $1,000,000/$2,000,000 (Per Claim/Aggregate). If Project Specific Insurance, the minimum <br /> extended reporting period shall meet the requirements of the Prime Contract, but shall not be less <br /> than three (3) years after completion of Subcontractor's Work or the statute of repose for the <br /> applicable state, whichever is longer. Subcontractor must advise of a material impairment of limit as a <br /> result of claims on work other than the stated project. A material impairment shall be considered any <br /> reduction in policy limits greater than twenty(20) percent. <br /> If any (a) engineering or design, (b) construction inspection, (c) survey work, or (d) any other <br /> miscellaneous professional services are included within the Subcontractor's scope of services, <br /> Subcontractor shall purchase and maintain professional liability insurance with minimum limits of <br /> $1,000,000 each occurrence and a $2,000,000 aggregate. If the policy is written on a Claims Made <br /> form and coverage expires or terminates at any time before three (3) years after acceptance or <br /> termination of Subcontractor's Work, Subcontractor shall obtain extended reporting period coverage <br /> ("tail cover"), for a period of not less than three (3) years from Subcontractor's last services or provide <br /> a project specific policy with a three (3)year extended reporting provision. <br /> VI. Rigger Liability <br /> $ 1,000,000 or Replacement Value of property being lifted, hoisted, rigged, whichever is <br /> greater(any one loss). <br /> VII. Watercraft Liability <br /> $5,000,000 in HulUProtection& Indemnity, including Jones Act Coverage, if applicable <br /> Equal Opportunity/Affirmative Action Employer <br />
