It starts on Page 69 <br /> of the application packet and if additional information is needed <br /> please let us know so we can provide it . I ' ll also point out <br /> that mining and reclamation activities have ended within 200 feet <br /> to the CPW fence and ther is no plan to move south the reclaimed <br /> slope . <br /> c. where such structure is a utility, the Applicant may supply a notarized letter, on <br /> utility letterhead, from the owner(s) of the utility that the mining and reclamation <br /> activities, as proposed, will have "no negative effect" on their utility. <br /> Not applicable. <br /> 5.
