Actions taken to respond to and correct the emergency situation or condition <br /> • Before midnight on August 12th, a barren solution pump failed, causing the barren solution tank <br /> on the west side of the building to fill faster than the remaining pumps could pump solution to the <br /> pad. CC&V personnel reacted quickly to balance flow across the plant. While the process was <br /> being brought back into equilibrium, approximately 5,500 gallons of barren solution went out the <br /> barren solution tank overflow (See photo 1) and spilled around the north side of SGADR within <br /> the containment of the Pregnant Solution Storage Area PSSA. <br /> b. Any known or adverse impacts to human health,property or the environment <br /> • No impacts to human health or the environment were observed or are expected to occur because <br /> all solution remained on liner and quickly infiltrated into PSSA. <br /> 1 <br />
