PERMIT#: M-1987-020 <br /> INSPECTOR'S INITIALS: SJM <br /> INSPECTION DATE: July 25,2018 <br /> OBSERVATIONS <br /> This was a routine inspection conducted by the Division as part of its monitoring of 112c permits. The <br /> operator, Patrick Sullivan, with Rio Grande County, was present during the inspection, and the site was not <br /> active on the day of the inspection. <br /> The required permit ID sign was observed posted at the entrance gate to the permitted area. Permit <br /> boundary markers were installed along the boundaries. <br /> There was no equipment or fuel at the site, and no debris or other contaminants. The excavated pit covers <br /> much of the permitted area, with most of the unmined portion along the southern edge. The pit depth varies <br /> from about 10 to 15 feet, and the floor is irregular. There is a small stockpile of pit run or gravel in the eastern <br /> end of the pit floor, from which some material has been recently removed. There were no other significant <br /> stockpiles of pitrun or processed gravel in the permit area. <br /> There is a large stockpile of topsoil on the unmined southern side of the site. It is estimated to contain about <br /> 3,000 to 4,000 cubic yards. This soil was imported to the site for use by the operator as an addition to the <br /> gravel product needed for maintaining county roads. It will not be used for top soiling the site for <br /> reclamation. This stockpile was not present at the time of the Division's last inspection. The Division <br /> recognizes this use of the soil, but the operator should review the permit to ensure whether the importation <br /> of offsite-generated material is an allowed activity under the permit. If it is not yet included in the permit <br /> plans, the operator should revise the permit (under a technical revision), and provide the information <br /> requested in Rule 3.1.5(9). This is not a problem at this time. <br /> All pit slopes were observed to be sloped at 3:1 or gentler, in accordance with the final slope gradients in the <br /> approved plan.
