PERMIT#: M-2018-027 <br /> INSPECTOR'S INITIALS: ECS <br /> INSPECTION DATE:August 6,2018 <br /> OBSERVATIONS <br /> This was a pre-operation inspection conducted by Eric Scott for the Division of Reclamation Mining and Safety <br /> (DRMS). DRMS met with permittee representative Andrew Rodriguez of Civil Resources LLC at approximately <br /> 0900 at the site entrance located on the west side of Highway 60 at the northeast corner of the site. DRMS <br /> joined Mr. Rodriguez and toured the site with him. The permit boundary was verified, and will primarily <br /> follow existing fence lines along the west and south side of the site, as well as the boundary along the west <br /> side of HWY 85 and 60. The northern permit boundary will run parallel to the north side of the existing access <br /> road. The permit boundary of the SE area of the site will follow the north side of county road 36, follow a 200' <br /> offset parallel to the railroad tracks, and follow the property boundary along the east side of the permit. The <br /> east and west sides of the SE portion of the permit do not follow any existing site features such as existing <br /> fences, therefore the permit boundary in these areas will need to be clearly delineated. <br /> In addition, the existing irrigation recharge pond located near the southwest corner of the site is outside of the <br /> proposed permit area and therefore the permit boundary in this area will also need to be clearly delineated. A <br /> DRMS estimate of the proposed permit area is included with this report (see attached map). <br /> The site is currently utilized primarily as general agriculture (hay/alfalfa field), but there are several gas/oil <br /> structures and other utilities within and adjacent to the permit area that will need to be noted during the <br /> operation. During this inspection it was noted that a new gas line was being installed parallel to the existing <br /> Suncor line that runs N-S through the west portion of the site as shown on the "existing conditions" map <br /> supplied with the application. <br /> No other issues were noted during this inspection, and DRMS left the site at approximately 10:30. <br /> Inspection Contact Address <br /> Andrew Rodriguez <br /> Civil Resources, LLC <br /> 323 5th Street <br /> Frederick,CO 80530 <br /> Page 2 of 3 <br />
