PERMIT #: M-2017-004 <br />INSPECTOR'S INITIALS: SJM <br />INSPECTION DATE: July 27, 2018 <br />OBSERVATIONS <br />This was a pre-operation inspection conducted by the Division of Reclamation, Mining and Safety (Division) in <br />response to the Construction Material Regular 111 Operation Reclamation operated by Four Corners <br />Materials. Matt Carnahan, representing the Operator, was present during the inspection. The permit was <br />approved in 2017 and no disturbance has taken place. Photographs taken at the site during this inspection are <br />included at the end of this report. <br />The South Fork Pit is located 5 miles east of South Fork, CO in Rio Grande County and will be accessed from <br />Colorado HWY 160. <br />The Mine Identification Sign was not in place and is noted as a problem at the beginning of this inspection <br />report. <br />No noxious weeds, equipment or fuel were stored on site. <br />The current site use is Rangeland, the post mining land use will be Rangeland. The permit area will include <br />30acres, of which 30 acres are expected to be mined. The Financial Warranty held by the Division is $75,000 <br />and is adequate at this time. There currently is a stockpile of asphalt and material stockpiles on site from an <br />adjacent pit. There are also Scale blocks and supports. This is considered a problem and is cited in the <br />beginning of the inspection report. <br />Mr. Carnahan mentioned that they did not get Rio Grande County approval for a project that this site was <br />permitted for.
