PERMIT #: M-2017-004
INSPECTOR'S INITIALS: SJM
INSPECTION DATE: July 27, 2018
OBSERVATIONS
This was a pre-operation inspection conducted by the Division of Reclamation, Mining and Safety (Division) in
response to the Construction Material Regular 111 Operation Reclamation operated by Four Corners
Materials. Matt Carnahan, representing the Operator, was present during the inspection. The permit was
approved in 2017 and no disturbance has taken place. Photographs taken at the site during this inspection are
included at the end of this report.
The South Fork Pit is located 5 miles east of South Fork, CO in Rio Grande County and will be accessed from
Colorado HWY 160.
The Mine Identification Sign was not in place and is noted as a problem at the beginning of this inspection
report.
No noxious weeds, equipment or fuel were stored on site.
The current site use is Rangeland, the post mining land use will be Rangeland. The permit area will include
30acres, of which 30 acres are expected to be mined. The Financial Warranty held by the Division is $75,000
and is adequate at this time. There currently is a stockpile of asphalt and material stockpiles on site from an
adjacent pit. There are also Scale blocks and supports. This is considered a problem and is cited in the
beginning of the inspection report.
Mr. Carnahan mentioned that they did not get Rio Grande County approval for a project that this site was
permitted for. If Four Corners Materials would like to mine this site, they will need to re -permit the site based
on a future government contract.
Responses to this inspection report should be directed to Stephanie Mitchell at the Division of Reclamation,
Mining and Safety, Grand Junction Field Office, 101 South 3rd Street, Room 301, Grand Junction, Colorado
81501, phone number (970) 242-5025.
