(:q )Q (IP3 ) ALtBffs
June 25, 2018
Colorado Division of Reclamation, Mining and Safety RECEIVED
Attention: Peter Hays
1313 Sherman Street, Room 215 JUN 2 2018
Denver, CO 80203
DIVISION OF RECLAMATION
MININrD SAFETY
RE: Inspection Report Cottonwood (M-2004-061) Corrective Action, Abatement Summary
Dear Mr. Hays,
This letter is in response to your inspection report dated May 25th, 2018 pertaining to the noted
problems contained within the report. One of the noted problems was for the PVC Pipe, which was
observed near the southwest corner of the site during the inspection. After further investigation, we have
concluded this pipe was installed by the predecessor for proper drainage of a natural spring so that the
water does not become impounded within the bermed area of the pit. Furthermore, the water is not
discharging offsite as evident during the inspection the water after its initial discharge from the PVC
pipe percolates back into the surface approximately 80 feet south of the protruding pipe.
The second problem indicated in the inspection report is signs & markers for the mining
boundary pertaining to the haul road in question. A survey of the boundary was staked and flagged (see
attached photos) as indicated in the photos the haul road is well within the boundary of the markers.
There is speculation as to the fill from the road that created a berm between the road and the boundary
Iine, whereas the toe of this material may be in contact with the boundary line. Staff has made note of
this observation and have determined to remove any material that may be in close proximity to the
boundary line and match to the existing surface contours, as well as seed any areas that are impacted
from this correction.
Please feel free to give me a call if you have any questions.
Kindest Regards,
Jon Mueller
Environmental Compliance Coordinator
Kilgore Companies - Colorado Companies
556 Struthers Avenue — Grand Junction, CO 81501
Main: 970.242.5370 - Direct: 970.255.2098 - Mob: 970.261.5782
