Pages enclosed for correction to include <br /> Kilgore Companies, LLC dba Elam Construction. <br /> o00" x Multi-Pit Mining Lease Agreement between Kilgore Companies LLC and A Et G <br /> F Partnership, LLP is required to be between Kilgore Companies LLC dba Elam <br /> Construction and Parkerson Brothers LLC and the land owner for demonstration of legal <br /> right to enter. <br /> G Structure Agreements were not provided for man-made structures, utilities and roads <br /> within 200 ft. of the affected land. See instructions enclosed for Structure Agreements. <br /> Affidavit of Authority for Russell A. Larsen to execute the financial warranty. <br /> Additionally, the Annual report, map and fee compliance is due by April 4, 2018. <br /> Please return the required documentation to the Division before April 27, 2018 to complete the <br /> application for approval. All completed permit documents are required to be submitted for <br /> consideration of your Request for Transfer of Mineral Permit and Succession of Operators <br /> Application. An incomplete application will be denied after the 60 day submittal request, if <br /> requested documents or information are not received.
