The <br />acceptable range of map scales shall not be larger than 1 inch = 50 feet nor smaller than 1 <br />inch = 660 feet. Also, that a map scale, appropriate legend, map title, date and a north arrow <br />shall be included. <br />-Please make sure that all provided maps and exhibits conform to the requirements of Rule 6.2.1(2), <br />including signing and dating by the person who prepared them. <br />EXHIBIT C - Pre -mining and Mining Plan Map(s) of Affected Lands (Rule 6.4.3): One or more maps <br />may be necessary to legibly portray the following information. <br />(a) all immediately adjacent surface owners of record; <br />-The contour map provided shows that the property to the north of the permit is owned by John <br />Vucetich, however the new map shows the property owned by Todd Potts. Please modify as needed. <br />Is the proposed permit boundary greater than 200' from the Vucetich property? <br />(d) the total area to be involved in the operation, including the area to be mined and the area of <br />affected lands (see definition of "Affected Land"); <br />-Line 3.1 of the application form states that 8.3 acres will be added to the existing permit area, <br />however the map provided shows the amended area as 11.5 acres. Please revise for consistency <br />where needed. <br />-Please remove all references to Potts Pit (M2004-005) from all maps; <br />-Line "d" of the supplied Exhibit C text lists the permitted area as 49 acres, please revise as needed; <br />and please rename the previous Exhibit C map so that it is not confused with the current submittal. <br />EXHIBIT D - Mining Plan (Rule 6.4.4): The mining plan shall supply the following information, correlated <br />with the affected lands, map(s) and timetables: <br />(d) The size of area(s) to be worked at any one time. <br />-Phase 1 will now be 30.7 acres (or 42 acres?) not 17 acres, please revise as needed. <br />-Please update. -revise section E as submitted to reflect the correct acreages and phases for the proposed <br />operation, including removing references to Phase III. <br />-Does the permittee wish to call this site an intermittent operation? <br />EXHIBIT E - Reclamation Plan (Rule 6.4.5): <br />(2) The Reclamation Plan shall include provisions for, or satisfactory explanation of, all general requirements <br />for the type of reclamation proposed to be implemented by you.
