RULE 2 - PERMITS <br />• beneath the road. Future monitoring of these monuments will be performed commencing one <br />month prior to the initiation of mining beneath any of the established monuments. The <br />locations of the monuments are shown on Map 2.05.6 -M1, Subsidence Monitoring. <br />A pre- mining survey of Boeddeker Reservoir #15 (0.5 ac. ft. stockpond) was conducted to <br />establish the surface elevation of the embankment and the invert elevation of the spillway at <br />the centerline of the embankment. Three rebar monuments were set to allow future <br />(mining/postmining) documentation. The location of the pond embankment and the <br />monuments are shown on Map 2.05.6 -M1, Subsidence Monitoring. <br />A preliminary premining profile of Grassy Creek, within the proposed mining zone, was <br />developed using the most current aerial topography available to PSCM. This profile will be <br />field verified once the spring runoff has subsided and the thalweg is accessible. The updated <br />and revised profile will be submitted once conditions allow physical surveying of the thalweg <br />to be completed. The location of the designated stream section and the preliminary profile <br />are shown on Map 2.05.6 -M1, Subsidence Monitoring. <br />The proposed monitoring program is a follows: <br />1. During each calendar quarter, a qualified Mining Engineer or Geologist will inspect all <br />entries and crosscuts mined during that quarter that are under less than 500 feet of <br />overburden for roof falls, pillar degradation, faults and water inflows. <br />• 2. All roof falls above the roof bolt anchor point will be recorded. <br />3. Any pillar degradation that reduces the pillar by more than 10% or increases the mine <br />opening by more than 10% will be recorded. <br />4. All faults that have a displacement of more than 12 inches will be recorded. <br />5. All water inflows of more that 5 gpm will be recorded. <br />6. Monitoring of the County Road 27 monuments will be performed commencing one <br />month prior to the initiation of mining beneath any of the established monuments. <br />7. The road monuments will be surveyed each May and October after mining beneath the <br />road. <br />8. The location and elevation of a concrete footer of the high voltage distribution tower <br />nearest each set of entries passing beneath the high voltage lines will be surveyed at least <br />one month prior to mining. The tower footer(s) will be surveyed each May and October <br />after mining beneath the tower. <br />9. The three monuments at Boeddeker Reservoir #15 will be re- surveyed once mining is <br />completed in the entry nearest the reservoir. <br />10. The sections of Grassy Creek over the entries will be re- surveyed if any failures in the <br />underlying entry are noted during the underground inspections. <br />11. The record of all findings will be submitted semi - annually once mining begins and will <br />include a written report and a map. <br />12. When the Division is satisfied with the results of this subsidence prediction, the reporting <br />will be discontinued with approval of the Division. <br />r <br />PSCM Permit App. 2.05 -105 Revision 04/19/10 <br />
