2.03.8 Permit Term Information
Below is the anticipated starting date and termination date of each phase of the underground
mining activities and the anticipated number of acres of surface lands to be affected, and the
horizontal and vertical extent of the proposed underground mine workings, for each phase of
mining and over the total life of the permit.

(1) Certificate of Insurance
A copy of the Certificate of Insurance is provided in Exhibit 2.03.9-E1, Insurance
Certificate. Future certificates will be kept on-site in the Compliance File.
2.03.10 Identification of Other Licenses and Permits
Below is a list of all other licenses and permits needed for the PSCM to conduct the proposed
underground mining activities. This list identifies each license and permit by type, name and
address of the issuing authority: the identification numbers of applications for those permits or
licenses, or, if issued, the identification numbers of the permits or licenses; and if a decision has
been made, the date of approval or disapproval by each issuing authority.
