No impacts to human health or the environment were observed or are expected to occur as this area has <br />not been exposed to process chemicals and no reports to Squaw Gulch leak detection systems have been <br />reported. <br />C. Monitoring and analyses that are necessary to evaluate the situation and corrective actions <br />At this time, daily inspections of the leak detection system and weekly monitoring of the underdrain <br />continue. To date, the leak detection system and the underdrain have been dry. Additionally, a charged <br />I2 inch header, located upgradient of the investigation area, will be inspected daily and is planned to be <br />taken off-line within approximately two weeks. Once the header is taken off-line, the normal inspection <br />schedule will resume. <br />CC&V and DBMS are working to determine the appropriate approach to determine the extent of damage <br />to the liner and the process whereby the liner is repaired and recertified. <br />e. Results of the Operator's investigation <br />The root cause of the liner tear was a slope failure that occurred in December when ore was being <br />stacked.
