Public Notice ran between 02/16/2018 <br />and 03/9/2018. Please find below the adequacy review issues that will need to be addressed prior to <br />the Division's approval of the revision. Please note these items were all originally provided to Moffat <br />County Mining, LLC. with the Midterm Review (MT7) dated March 2, 2016. The Division recognizes that <br />some of these items may have been updated or are no longer applicable. However, for continuity and <br />tracking of item resolution, the original MT7 numbering has been maintained. Please refer to the bold <br />italic below adequacy items for updates the Division is currently aware. <br />The Division is required to issue its proposed decision for this minor revision by 6/12/2018. Please <br />provide responses with adequate time for the Division to review items prior to the decision date. <br />Please address the following items: <br />2.03.4 Identification of Interest <br />1. In accordance with Rule 2.03.4(3)(a), for each person (officer and director of the entities) who <br />owns or controls the applicant, please provide their address, social security number (optional) <br />and employer identification number. <br />2. The information required by Rule 2.03.4(3)(d) and (e) will need to be provided. For each person <br />who is the owner and controller of the applicant, please supply all the information required under <br />this rule for all owned or previously owned or controlled surface coal mining and reclamation <br />operation in the United States within the five years preceding the date of the application. <br />3.
