PERMIT #: M-1976-018 <br />INSPECTOR'S INITIALS: ECS <br />INSPECTION DATE: March 28, 2018 <br />INSPECTION TOPIC: Gen. Compliance With Mine Plan <br />PROBLEM/POSSIBLE VIOLATION: Problem: There is a large amount of treated timber stockpiled within the <br />permit at this time (Photos 5-6). The operator will need to submit a TR to describe how this material will be <br />utilized as part of the approved mining plan, or remove it from the permit by the corrective action date. <br />CORRECTIVE ACTIONS: The operator shall submit a Technical Revision, with the required $216 revision fee, to <br />update and clarify the current approved mine plan to reflect existing and proposed activities or remove the <br />timber from the permit area by the corrective action date. <br />CORRECTIVE ACTION DUE DATE: 6/01/18 <br />OBSERVATIONS <br />This was a routine monitoring inspection for the North Weld Gravel permit, an 80 acre 112c sand and gravel <br />operation, conducted by Eric Scott of DRMS. DRMS met permittee representative LeRoy Bellmore and <br />observed the site with him at approximately 10:00. <br />The permit area encompasses 80 acres, with a maximum disturbance of 40 acres, and is located on the <br />southwest corner of the intersection of WCR 96 and WCR 39. The site identification sign was observed at the <br />existing site entrance location (photo 1), and the extent of the site boundary was marked with posts. <br />The site is active although no mining was taking place during this inspection. Approximately 33 acres within <br />the permitted area have been disturbed at this time, and the topsoil and overburden have been stockpiled along <br />the perimeter of the excavated areas. <br />Issues noted during this inspection that will require corrective action have been noted in the previous section of <br />this report and additionally documented with the following photos.
