PART <br />Page 24 of 47 <br />Permit No.: C00000213 <br />At the request of the Division, the permittee shall show proof of the accuracy of any flow -measuring device used in obtaining <br />data submitted in the monitoring report. The flow -measuring device must indicate values within ten (10) percent of the actual <br />flow being measured. <br />4. Extra Monitorina <br />If the permittee, using an approved analytical method, monitors any parameter more frequently than required by this permit, <br />then the results of such monitoring shall be included in the calculation and reporting of the values required in the Discharge <br />Monitoring Report Form (DMRs) or other forms as required by the Division. Such increased frequency shall also be indicated. <br />S. Adverse Weather Conditions <br />When adverse weather conditions prevent sample collection according to the relevant monitoring schedule, the permittee must <br />take a substitute sample, as possible, during the remaining monitoring period; for stormwater, the permittee must take a <br />substitute sample during the next qualifying storm event. Adverse conditions are those that are dangerous or create <br />inaccessibility for personnel, such as local flooding, high winds, winter weather, or electrical storms. <br />K. REPORTING AND RECORDKEEPING <br />1. Routine Reporting of Data- Discharge Monitorina Report <br />The permittee shall report the data gathered in compliance with this permit on a monthly basis. Reporting of all data gathered <br />shall comply with the requirements of the GENERAL MONITORING AND SAMPLING REQUIREMENTS section and the REPORTING <br />AND RECORDKEEPING section of this permit. The permittee shall summarize monitoring results for each month and report on <br />Division approved discharge monitoring report (DMR) forms (EPA form 3320-1). <br />The permittee must submit these forms either by mail, or by using the Division's Net -DMR service (when available). If mailed, <br />one form shall be mailed to the Division, as indicated below, so that the DMR is received no later than the 28th day of the <br />following month (for example, the DMR for the first calendar quarter must be received by the Division by April 28th). If no <br />discharge occurs during the reporting period, "No Discharge" shall be reported. <br />The original signed copy of each discharge monitoring report (DMR) shall be submitted to the Division at the following <br />address: <br />Colorado Department of Public Health and Environment <br />Water Quality Control Division <br />WQCD-P-B2 <br />4300 Cherry Creek Drive South <br />Denver, Colorado 80246-1530 <br />The Discharge Monitoring Report forms shall be filled out accurately and completely in accordance with requirements of <br />this permit and the instructions on the forms. They shall be signed by an authorized person as identified in Part I.D.8. <br />2. Annual Biosolids Report <br />The permittee must submit these forms either by mail, or by using the Division's Net -DMR service. If mailed, one form shall be <br />mailed to the Division, as indicated below, so that the DMR is received no later than the 28th day of the following month (for <br />example, the DMR for the first calendar month must be received by the Division by February 28th). If no discharge occurs during <br />the reporting period, "No Discharge" shall be reported on the DMR. <br />The signed copy of each discharge monitoring report (DMR) shall be submitted to the Division at the following address: <br />Colorado Department of Public Health and Environment <br />Water Quality Control Division <br />WQCD-P-B2 <br />
