• <br /> (Page 2) <br /> MINE ID#OR PROSPECTING ID # M-1977-438 (revoked) <br /> INSPECTION DATE 3-11-2003 INSPECTOR'S INITIALS SSS <br /> OBSERVATIONS <br /> This inspection was conducted to determine if any hazardous materials are present(abandoned) on or within the mine <br /> prior to sealing by DMG. The permit was revoked and the financial warranty forfeited by the Mined Land Reclamation <br /> Board on March 26, 2003. <br /> Cotter Corp. of Nucla allowed use of 3 Radon daughter's respirators to be used and returned to Cotter upon completion of <br /> the mine inspection. CMX-270 and LTX-412 multi-gas detectors were used to determine continuous Oxygen, CO, CH4 <br /> and NO2 readings while within the mine. <br /> The first area inspected was the underground shop. One half-empty 5 gallon can of hydraulic fluid and miscellaneous <br /> parts for mining equipment. No explosives or other hazardous materials were noted. A link of the chain securing the shop <br /> gate was cut and replaced with a master lock. The original lock was left in the loop. A key for the DMG lock can be <br /> obtained at the Grand Junction Field Office. <br /> Mine entry, at the lower portal, at approx. 9:30 AM. 02 at portal 20.7% no other gases detected. Mine examination <br /> revealed numerous (>20)empty containers (boxes) of explosive materials. One empty paper cartridge was found but no <br /> explosive material was within the paper. RE: picture 1. One open bag of exposed ammonium nitrate blasting agent was <br /> discovered. This bag was only about 1/3 original weight (estimated) and is fully exposed to the elements within the mine. In <br /> John's opinion this is not a hazard due to it's location within the mine and deterioration. The entire mine was not inspected <br /> due to hazardous roof conditions and water over knee deep. A few(3-5) partially filled 5 gallon cans ere found, but not <br /> investigated to determine there contents. 02 at FPA was 20.6%. Two mouse-sized brown bats were noted within 200 feet <br /> of the portal. <br /> Approximately 1:30 PM the small uppermost portal was entered for inspection. Mine examination revealed numerous <br /> (>50)empty containers (boxes) of explosive materials. There was approximately 50 ft. of safety fuse discovered. Many <br /> small pieces varying in length. This material was brought outside and ignited on the surface. RE: picture 2. There was also <br /> one bootleg hole discovered within the mine but again this in John's opinion is not a hazard due to its location within the <br /> mine. (more hazardous to attempt to remove) <br /> The Pickett Corral portal on the main bench was gated, locked and was not inspected.
