File No. M-2018-007• Pursuant to Rule 1.4.1(8), the Division has determined the application contains deficiencies which prevent the application 1'roin being considered tiled. APPLICATION FORM 1. On the third page of the application form, the primary mine entrance location is provided by Universal Transverse Mercator (UTM) coordinates, however, it appears these coordinates do not accurately depict the actual location of the mine entrance. The Applicant provided NAD 27, Zone 12 & 13, Eastirrg 130475322, Aror•thing 4318454, while it appears it should have been NAD 27, Zone 13. Ectmirrg ; 75322, Northing 43184-0. Please revise the primary aline entrance coordinates and submit E a replacement 1�a�oe three of the application form as well as making the same change to the Exhibit A materials. The Applicant may also wish to provide the Latitude: l.on�,itude coordinates of these saitic U'l-N'I coordinates which would be 39' 00' 59.91" N 105° 17' 06,1 W. 2. Responsibility #10 on page six of the application form has been initialed. This is likely in error as the applicant appears to not be a joint venturelpartnership business.
