The spill occurred early morning February <br /> 5, 2018. <br /> The following points address the requirements of Rule 8, section 8.2.3: <br /> a. Actions taken to respond to and correct the emergency situation or condition <br /> • The spill was observed by the mill control room at approximately 2:00am. The spill occurred <br /> while personnel were in the process of cleaning out the secondary containment around the <br /> Concentrate Thickener. A hose became detached from a portable diaphragm pump used in <br /> the cleanout, resulting in approximately 100 gallons of slurry leaving secondary containment. <br /> • A skid steer was used to over excavate the impacted soil. Depth of recovery was <br /> approximately 6 inches (see photo 1). Slurry was recovered from the first few inches of soil; <br /> the underlying soil was not wet; therefore, CC&V can ensure that all contaminated soil was <br /> removed. <br /> • Approximately 5 cubic yards of slurry and soil were excavated with a skid steer; the material <br /> will be mixed with leach ore at the Load out Bin. <br />
