The site is currently broken into 9 areas where mining will take place. Summit Brick will use <br />these areas to determine what structures will need to be inspected prior to blasting occurring in them. <br />In most cases mining will start on the east side of any given area and work west across the face of the <br />Area. Prior to opening anew Area, a survey will be done to determine all habitable structures within <br />one-half mile of the new mining area. <br />The survey will be done in three steps, In the first step, Summit Brick will use aerial photographs, <br />county assessor data, and maps to prepare a list of potentially affected structures. Landowner <br />interviews may also be conducted to identify structure located within the half mile radius of any give <br />blasting area. In the second step, Summit Brick will send the structure owner notifications by certified <br />mail to the owners of the structures at least 30 days prior to any blasting being conducted within one- <br />half mile of the structure. The survey will be complimentary to the neighbor. Included in the notification <br />will be a description of the process that will take place on the day of the inspection, what the inspector <br />will be looking for, and how the inspection will be documented. Summit Brick and Tile Company will <br />recommend that the landowner collect any documents relevant to the structures that will be evaluated; <br />these may include date and details on the structure construction or any other information they would <br />like to be considered during the inspection. Also included with the notification will be a questionnaire <br />that each neighbor will complete and will be asked to return it within 20 days of the notice mailing date. <br />This form will be used as consent to complete the survey or as statement by the landowner declining the <br />survey. If no questionnaire is returned within the 20 day reply period the lack of response will be <br />deemed to indicate they declined participation in the survey. In the third step, the owners that choose <br />to participate in the survey will be contacted by Summit Brick to make arrangements for a <br />representative of Summit Brick and Tile Company to perform the inspection. <br />Structures that will be considered in the pre -blast survey include houses or any other dwelling, <br />garages, barns, or outbuildings. This list is not all inclusive, and during the survey, the landowner may <br />request that other structures on the property be surveyed. <br />On the day of the survey the representative from Summit Brick and Tile Company will arrive on <br />the property, introduce themselves and conduct the inspection. Any available documents will be <br />reviewed at the start of the survey. The consulting representative will have a still photo camera and a <br />video camera. The representative doing the inspection will photograph and video document, with voice <br />comments, the exterior and interior of all structures located on the property. They will document the <br />existing condition of all walls, doors, ceilings, windows and floors. The survey will include all levels of the <br />structure including the basement and foundation. Areas that cannot be accessed during the inspection <br />will be noted. The current conditions of the structure will also be recorded in the form of notes and <br />sketches on a standard inspection form used by the company. <br />The landowner will be encouraged to accompany the inspector, point out items to be included <br />in the documentation, and discuss any items included in the survey and report. The landowner may also <br />document the survey using their own photographs, video camera, and notes. Summit Brick and Tile <br />Company will also request copies of any landowner photographs and notes to ensure a complete file of <br />the survey is compiled. <br />Upon the completion of the pre -blast surveys the company will assemble all documentation <br />collected and prepare a report stating the findings. This report along with all photographic and video <br />documentation will be placed on DVD and copies will be generated. One copy will be given to the <br />property owner, one copy will be kept by Summit Brick and Tile Company and the last copy will be filed <br />with the Colorado Division of Reclamation, Mining, and Safety (DRMS). <br />
