Environmental Protection Facility Certification <br />Henderson Mill — Permit No, M-1977-342 <br />Technical Revision 28 <br />Ute Park Extraction Wellfield Project — Phase III <br />The Ute Park Extraction Wellfield Project -- Phase III was approved as Technical Revision No. 25 to MIII <br />Permit No. M-1977-342 on April 25, 2015. This project is located at Climax Molybdenum Company's <br />Henderson Mill In Grand County. The project involved the construction of two extraction wells, one <br />monitoring well, and support infrastructure, including pipeline, vault, electrical power line, and <br />instrumentation at locations down gradient from 1 Dam at the Mill. <br />The Henderson Mill and contractors have substantially completed the construction of the project and it <br />has been tested and determined to be functional. The systems were designed and installed to meet <br />applicable electrical codes and Henderson design requirements. <br />Modifications to the initial design have been reflected in the record drawings for MLGW-26, MLEX-4 and <br />MLEX-5. <br />FMI EIS group was responsible for the design, coordination, and implementation of the electrical <br />systems. The project was completed in general conformance with the approved design as submitted to <br />the Division, of Reclamation, Mining, and Safety.
