In order to accomplish this,you must submit a request to amend <br /> the Mined Land Reclamation Board Order for Violation number MV-2017-049 to "allow for the <br /> submission of a regular Hard Rock and Metals Mining 110 Reclamation Permit, rather than a <br /> 110 Reclamation only permit within 90 days of consent approval." This request must be made <br /> in writing, signed, notarized and submitted to the Board Secretary, Camille Mojar, at 1313 <br /> Sherman Street, Room 215, Denver CO, 80203. <br /> Please find the enclosed 110 Reclamation Permit Application to be submitted. Please read this <br /> carefully and submit the application and fee in accordance with the applicable Rules and <br /> Regulations detailed in the application packet.
