OPERATING & MAINTENANCE REQUIREMENTS <br />5. The owner or operator shall develop an operating and maintenance (O&M) plan, along with a recordkeeping <br />format, that outlines how the applicant will maintain compliance on an ongoing basis with the requirements <br />of this permit. Compliance with the O&M plan shall commence at startup. Within sixty (60) days after <br />issuance of this permit, the owner or operator shall submit the O&M plan to the Division. Failure to submit <br />an acceptable operating and maintenance plan could result in revocation of the permit. (Reference: <br />Regulation No. 3, Part B, III.E.) <br />This point was not reported or permitted prior to being noted during the 2012 inspection. The <br />source reported that they have always had an O&M procedure, but formally submitted this plan <br />following Initial Approval Permit issuance. The O&M Plan was approved on March 6, 2014 and <br />the source has maintained all maintenance and inspection records as required. The source also <br />created a detailed recordkeeping format that includes all appropriate monthly and yearly <br />throughput and emissions tracking information. The source is in compliance with this condition. <br />COMPLLINCE TESTING AND SAMPLING <br />Initial Testing Requirements <br />6. Within sixty (60) days after issuance of this permit, the owner or operator shall demonstrate compliance <br />with Condition 10, using EPA Method 9 to measure opacity from the Rock Dust Silo and the baghouse. <br />Sources not Subject to opaci1y readings of an NSPS subpart: <br />This measurement shall consist of a minimum twenty-four consecutive readings taken at fifteen second <br />intervals over a six minute period. (Reference: Regulation No. 1, Section II.A.1 & 4) <br />The source conducted initial EPA Method 9 observations as part of the Initial Approval permit <br />requirements, prior to issuance of this Final Approval permit. Source is in compliance. <br />ADDITIONAL REQUIREMENTS <br />The AIRS ID number shall be marked on the subject equipment for ease of identification. (Reference: <br />Regulation No. 3, Part B, III.E.) (State only enforceable) <br />The AIRS ID number is marked on the silo as required. Source is in compliance. <br />A Revised Air Pollutant Emission Notice (APEN) shall be filed: (Reference: Regulation No. 3, Part A, <br />Section II.C.) <br />e. Annually whenever a significant increase in emissions occurs as follows: <br />For any criteria pollutant: <br />For sources emitting less than 100 tons per year, a change in actual emissions of five tons per <br />year or more, above the level reported on the last APEN submitted; or <br />For volatile organic compounds (VOC) and nitrogen oxide (NOx) sources in an ozone non - <br />attainment area emitting less than 100 tons of VOC or nitrogen oxide per year, a change in actual <br />2015 Inspection AXW <br />m <br />051001 S-INSP-201 S Page 26 of 37 <br />
