SOURCE COMPLIANCE HISTORY 2012 — Division inspection determined source to be in violation for failing to submit APENs and obtain permits for the rock dust silo (had been present for many years) and NSPS Subpart IIII engines (no longer a requirement if emissions are below reporting thresholds). The source was also determined to be in violation for failing to operate in a manner consistent with good air pollution control practices for minimizing emissions, as visible emissions were observed from an improperly installed baghouse access door during rock dust silo loading. These issues were corrected following the inspection.
