SOURCE COMPLIANCE HISTORY <br />2012 — Division inspection determined source to be in violation for failing to submit APENs and obtain permits <br />for the rock dust silo (had been present for many years) and NSPS Subpart IIII engines (no longer a requirement if <br />emissions are below reporting thresholds). The source was also determined to be in violation for failing to operate <br />in a manner consistent with good air pollution control practices for minimizing emissions, as visible emissions <br />were observed from an improperly installed baghouse access door during rock dust silo loading. These issues <br />were corrected following the inspection. All violations were included in Case #2013-126 and were settled with the <br />2015 Inspection mW <br />0510015-INSP-2015 Page 6 of 37 <br />Sylvester Gulch: Joy model 120-65-880, 1500 hp (exhaust) <br />Shaft #1 — fan #1: TLT Babcock model GAF 30-15-1, 2500 hp (intake) <br />Shaft #2 — fan #1: TLT Babcock model GAF 30-15-1, 2500 hp (intake) <br />Shaft #2 — fan #2: TLT Babcock model GAF 30-15-1, 2500 hp (intake) <br />Shaft #3 — fan #1: Joy model MF -65, 1500 hp (intake — not currently operated) <br />Ventilation fans are uncontrolled. All processes in line after the Series 500 Crusher are <br />considered wet processes and are APEN exempt. <br />One (1) Cummins, Model QSK60-G9, S/N: 33167177, diesel fueled, reciprocating <br />internal combustion engine, rated at 3,251 bhp, supplying power to an approximately <br />2,400 kW generator. This engine is equipped with an aftercooler for emission control. <br />The unit is subject to NSPS Subpart IIII, and its serial plate lists that it is a `Stationary <br />021 <br />l OGU1130 <br />Tier F engine. This emergency generator provides backup power to the Shaft #2 fan <br />such that miners will receive adequate ventilation air during an electrical failure at <br />the mine. MCC submitted a cancellation notice on 10/26/2015 for the small generator <br />associated with this point that is used to power the fan during startup to prevent <br />stalling. <br />022 <br />13GUI462 <br />One (1) pneumatically loaded rock dust silo equipped with a pulse jet baghouse to <br />control particulate matter emissions. <br />One (1) diesel fueled reciprocating internal combustion engine, rated at 50 hp, <br />supplying power to the emergency backup generator for the main office building <br />023 <br />13GUI463 <br />server. The unit was manufactured in 2009 and installed at MCC in 2010, and is thus <br />subject to NSPS Subpart IIII. MCC Submitted a cancellation notice for this point on <br />10/26/2015. <br />Emergency Generator at Deer Creek Escapeway Hoist. Make: Cummins, Model: <br />024 <br />14GU0697.XP <br />GTA19G1, SN: 25335469. This engine has a rating of 421 BHP and is propane <br />fueled.
