Miranda Kawcak of Seneca Property, LLC accompanied the inspection. The weather was clear and the
site was dry at the time of the inspection. The site is a surface mine currently in reclamation. No activities were
being conducted on the site at the time of the inspection. Hunters were observed during the inspection with
campers and utvs parked at the shop area. No permitting activities are on-going.
HYDROLOGIC BALANCE - Rule 4.05 Drainage Control 4.05.1, 4.05.2, 4.05.3; Siltation Structures 4.05.5,
4.05.6; Discharge Structures 4.05.7, 4.05.10; Diversions 4.05.4; Effluent Limits 4.05.2; Ground Water
Monitoring 4.05.13; Surface Water Monitoring 4.05.13; Drainage – Acid and Toxic Materials 4.05.8;
Impoundments 4.05.6, 4.05.9; Stream Buffer Zones 4.05.18:

Pond 006 was stable, holding water, and discharging at the time of the inspection. The slump on the south side of the pond remains up and away from the pond at
the time of this inspection.

Pond 015 embankments were stable and well vegetated. The pond displayed a pool of water and was not
discharging at the time of the inspection. No problems were noted with the primary or emergency spillways.
Prior to this inspection it was noted that the Pond 015 primary riser pipe was damaged. The pipe had rusted
through to allow water to enter the pipe approximately 2 feet below the top of the riser. A small amount of initial
erosion had begun around the pipe and into the embankment. The pipe has been repaired and the repair work was
adequate to allow the riser to function as designed. No problems were noted.

Pond 009 embankments were stable and well vegetated.
