This State-approved form makes reference <br /> to "...the above-listed structure(s)...",but no structures are listed on the face of the form. The <br /> form also says that any alterations will cause the form to be voided. To avoid any ambiguity, I <br /> want to make it clear that the Town-owned structures in question include a roadway surface on <br /> WCR 13, a small-diameter water line adjacent to WCR 13 and a twenty-four-inch water main <br /> adjacent to Colorado State Highway 392.
