(CBCI) to Caldera. The property conveyed by the Deed <br /> includes all the surface and mineral rights previously owned by CBCI. <br /> Also included herein, is a letter from CBCI providing the Division with notice that the the $25,000 <br /> portion of the Financial Warranty for the Permit has been assigned to Caldera; along with a Financial <br /> Warranty Check for Deposit in State Treasury form referencing the same, and an affidavit of authority <br /> attesting my signatory authority on behalf of Caldera. <br /> Lastly, please note that the applicant/operator, permitting, inspection, and landowner contact information <br /> for Caldera is as follows: <br /> Mike Thompson,Manager <br /> Caldera Mineral Resources, LLC <br /> P.O.
