Stormwater Management Plan <br />FEDERAL, STATE, OR LOCAL STORM WATER OR OTHER <br />ENVIRONMENTAL INSPECTOR SITE VISIT LOG <br />NOTES to General Contractor: <br />Upon completion of a storm water or erosion & sediment control agency inspection or site visit the General <br />Contractor shall: <br />1) Call the George M.L. Robinson (720-641-2534) or Storm Water Administrator *see <br />below <br />2) Complete an Inspection Report <br />3) Transfer all noted deficiencies to the Inspection Report and resolve as appropriate <br />4) Complete the inspector site visit log <br />5) Submit agency inspection reports if/when received to Mr. Robinson at (303-832- <br />7469) or georgerobinson@r2incorporated.com, or to the Storm Water Administrator <br />6) Take photos of any area or item that is photographed by the inspector. <br />The General Contractor is not required to complete the above noted steps if the agency inspection is not <br />storm water related (i.e., MS14A, Fire Marshal, etc.); however, if aLny agency representative comments on <br />storm water -related BMPs while on-site the above noted steps must be followed. <br />*When calling Mr. Robinson or the Storm Water Administrator to report an agency inspection the <br />General Contractor should leave the following information: <br />1. Caller (full name & firm) <br />2. Caller Telephone Number <br />3. Inspection Date <br />4. Name of Agency & Agency Representative (Inspector) <br />5. Was the inspection storm water / erosion -related (Yes or No)? <br />5b. If no, what was inspected? <br />Note: If the inspection was not storm water -related remaining questions can be skipped. <br />6. Deficiencies or items requested for improvement (add additional lines as needed): <br />1) <br />2) <br />7. Were all issues noted by the Inspector, from question 6, added to the Inspection Report as deficiencies <br />(Yes or No)? <br />7b. If no, add deficiencies to Inspection Report and resolve. <br />8. Did the inspector provide a report while on site (Yes or No)? <br />8b. If yes, submit the report to Mr. Robinson or the Storm Water Administrator by fax or email. <br />8c. If no, submit a report, if received, to Mr. Robinson or the Storm Water Administrator by fax or <br />email. <br />9. Was a Notice of Violation or similar warning issued or verbally referenced by the Inspector (Yes or No)? <br />9b. If yes, what specifically did the Inspector find in violation? <br />10. Is an inspection response due (Yes or No)? <br />10b. If yes, when is the response due date? <br />11. Did the Inspector reference a follow-up inspection (Yes or No)? <br />I lb. If yes, when is the follow-up inspection? <br />12. Was the Agency Inspection Log in the SWMP Binder updated (Yes or No)? <br />12b.
