COLORADO <br /> Division of Reclamation, <br /> Mining and Safety <br /> :.L,: . - <br /> September 11, 2017 <br /> Dennis Luke <br /> CEMEX, Inc. <br /> 13301 53`d St. <br /> Longmont, CO 80503 <br /> Re: CEMEX Sandstone Quarry,Permit No. M-1977-361 <br /> Documentation of Corrective Actions Received <br /> Mr. Luke: <br /> On September 6, 2017, the Division of Reclamation, Mining, and Safety (Division) received your <br /> Corrective Action response regarding the problem identified during the inspection conducted at the <br /> CEMEX Sandstone Quarry on March 22, 2017. The status of the Corrective Action associated with this <br /> problem is as follows: <br /> Corrective Action Response: The operator submitted a letter (on April 25, 2017) signed by Bradley <br /> Shane Wilson confirming the offsite disturbance is not associated in any way with the permitted <br /> operation. The Division later received a notarized letter signed by Michael Loukonen, adjacent <br /> landowner, attesting to the use of CEMEX property for processing stone by Loukonen Brothers Stone <br /> Company via an agreement between the two parties.
