The <br /> text provided does not identify or provide any information for any of these wells, and in fact, lists a well ID <br /> that does not appear to be shown on the provided maps at this time. Please revise for completeness and <br /> consistency between text and maps. <br /> EXHIBIT L - Reclamation Costs (Rule 6.4.12): (1) All information necessary to calculate the costs of <br /> reclamation must be submitted and broken down into the various major phases of reclamation. You must <br /> provide sufficient information to calculate the cost of reclamation that would be incurred by the state. <br /> The current bond for the site was calculated for reclamation of all exposed groundwater utilizing the previous <br /> DRMS policy of bonding for 20% of total slurry wall costs utilizing a $3 per sq./ft. rate for estimating total <br /> slurry wall installation costs. This policy has been rescinded and the permittee will now be required to submit <br /> financial warranty for the full cost of lining or backfilling as appropriate for the approved reclamation plan. <br /> This should also include cost for installation of appropriate flood control structures for reservoirs in the flood <br /> plain. <br /> Please provide a detailed reclamation estimate including tasks and unit rates for completing the reclamation <br /> as proposed in Exhibits E and F. In addition to grading, topsoil replacement, and revegetation,this estimate <br /> shall include costs for draining, re-sloping, fully lining or backfilling all exposed groundwater areas, and <br /> installation of flood control structures as appropriate. If the SEO has approved the liner for the eastern"slurry <br /> lined pond"please provide a copy of the approval documentation. <br /> EXHIBIT S - Permanent Man-Made Structures (Rule 6.4.19): Please note that roadways and above- <br /> ground or underground utilities (if present) within 200 feet of the proposed affected area are considered <br /> permanent man-made structures. In accordance with Rule 6.4.19, when mining operations will adversely <br /> affect the stability of any significant, valuable and permanent man-made structure located within 200 feet of <br /> the affected area,the applicant may either: <br /> a.)Provide a notarized agreement between the applicant and the person(s)having an interest in the structure, <br /> that the applicant is to provide compensation for any damage to the structure; or <br /> b.) Where such an agreement cannot be reached, the applicant shall provide an appropriate engineering <br /> evaluation that demonstrates that such structure shall not be damaged by activities occurring at the mining <br /> operation. <br />
