PERMIT #: M-1977-567 <br />INSPECTOR'S INITIALS: SJM <br />INSPECTION DATE: August 15, 2017 <br />OBSERVATIONS <br />This was a routine inspection conducted by the Division as part of its monitoring of Construction Materials <br />110c permits. The Mr. Elvie Samora with Saguache County Road & Bridge accompanied me on the inspection. <br />The site was not active on the day of the inspection. <br />The permit ID sign was observed near the entrance gate to the permitted area. Permit boundary markers, <br />consisting of fencelines, were present along the south and east permit boundaries. The NW corner marker <br />was re -installed in the correct location since the last inspection in 2014. <br />There has been very little recent activity, mostly limited to removal of stockpiled pitrun. There are several <br />small existing stockpiles in the pit. The operator is reminded to include all site activities in the annual reports <br />each year, including stockpiling, processing, excavation, hauling, reclamation, and weed control. There was <br />some slope grading in the past on the south and east walls of the pit, and the respreading of topsoil there. <br />The other pit walls are mostly steeper than what is required in the approved reclamation plan, and will require <br />grading to reduce them to the final gradient. The plan calls for 3:1 slopes on the west, south and east sides, <br />and 5:1 on the north side. <br />The site is accessible to cattle (presumably the landowner's livestock). The landowner feeds cattle in the area <br />lying to the west of the pit. The operator is reminded that when any phases of revegetation are to be carried <br />out as part of the final reclamation on this permit, the operator must ensure that the seeded areas are fenced <br />to exclude livestock, at least for the period needed to establish the vegetation. <br />There was an asphalt pile in the permit area. An Operator may backfill structural fill material generated within <br />the MLRB permitted area into an excavated pit within the permit area as provided for in the MLRB Permit. If <br />an Operator intends to backfill inert structural fill generated outside of the approved permit area, it is the <br />Operator's responsibility to provide the Office notice of any proposed backfill activity not identified in the <br />approved Reclamation Plan via a Technical Revision. The Notice to the Office shall include but is not limited to: <br />(a) a narrative that describes the approximate location of the proposed activity; <br />(b) the approximate volume of inert material to be backfilled; <br />(c) a signed affidavit certifying that the material is clean and inert, as defined in Rule 1.1(20); <br />(d) the approximate dates the proposed activity will commence and end, however, such dates shall not be an <br />enforceable condition; <br />(e) an explanation of how the backfilled site will result in a post -mining configuration that is compatible with <br />the approved post -mining land use; and <br />(f) a general engineering plan stating how the material will be placed and stabilized in a manner to avoid <br />unacceptable settling and voids. <br />There was no equipment, fuel, or debris on the site. There were no noxious weeds. <br />Regarding the annual map submitted to the Division, staff recommends that the topsoil stockpile locations be <br />included on the map as well as a North arrow. <br />Page 2 of 5 <br />
