Examples of EFRI's continued commitment include the following: <br /> • Submittal of Annual Reports to DRMS, including payment of Annual Fees and renewal of the <br /> Financial Warranty instrument in the amount of$375,078; <br /> • Submittal of Annual Reports to the Utah Department of Oil, Gas and Mining ("DOGM"), <br /> including payment of Annual Fees and renewal of Financial Warranty instruments; <br /> • Implementation of a quarterly hydrological monitoring program consistent with the DRMS- <br /> approved Environmental Protection Plan and U.S. Bureau of Land Management (`BLM") <br /> approved Plan of Operations, and submittal of Quarterly Hydrological Monitoring Reports to <br /> DRMS and BLM; <br /> • Renewal of Colorado (Water) Discharge Permit System Permit No. CO0047562 through the <br /> Colorado Department of Public Health and Environment (CDPHE) in 2015; <br /> • Amendment of a Road Improvement and Maintenance Agreement with Mesa County in 2016; <br /> and <br /> • Commencement of an additional one-year baseline hydrological monitoring program during <br /> 2017 to expand the number and location of monitoring points at the request of an interested <br /> stakeholder. <br /> (d) for an in situ leach mining operation, a description of the ground water monitoring <br /> and pumping regime that will be maintained during the period of cessation of <br /> operations and a schedule for reporting monitoring data. <br /> EFRI Response: <br /> Not Applicable—The Whirlwind Mine is a conventional underground uranium and vanadium mine and <br /> does not use in situ leach technology. <br /> If you have any questions or need additional information regarding this request, please contact me at <br /> 303-389-4132 or sbakken@energyfuels.com. <br /> Sincerely, <br /> -_�'� _._........� <br /> ENERGY FUELS RESOURCES(USA) INC. <br /> Scott A.
