Everist, Inc. <br /> Dear Division: <br /> L.G. Everist, Inc., respectfully submits the Annual Report for the Firestone Gravel Resource, DRMS <br /> Permit# M-1996-052, along with a check for$791.00 to cover the Annual Fee. The Annual Report <br /> includes a Mining Map for a visual representation of the affected areas. <br /> The affected area remains the same since the last annual report. We have continued mining in the <br /> western half of the site, and continue to use the settling pond in the southwest corner of the site. We <br /> have continued our weed control and management with multiple times mowing and spraying throughout <br /> the growing seasons. <br /> The construction of a concrete batch plant was completed at the beginning of 2017. The batch plant is <br /> located along the west side of the site in the plant area, and shown in our approved permit materials, <br /> and it is situated between the main entrance and the mine processing plant. <br /> Attached as required is Groundwater Monitoring Data for the site from 2016 and 2017-YTD, and a copy <br /> of the Monitoring Well map. Analysis of the groundwater monitoring data showed that groundwater <br /> levels have remained in compliance. MW14 was out of range 1 month in 2016, but this well is interior <br /> to our operation, so will not adversely affect any neighbors. And MW03 was out of range 1 month in <br /> 2017, but back in range the following month.
