Recorded at_o'clock_M <br /> ' Reception # Recorder <br /> Lee Hill Project <br /> CONSENT FOR RIGHT OF ENTRY FOR RECLAMATION ACTIVITIES <br /> Harry&Astrid Covey hereby represents to the State of Colorado, Department of Natural Resources, Division of <br /> Reclamation, Mining and Safety, that the undersigned is the owner, one of the owners, or represents the owner of <br /> the real property described in the table below, located in the County of Boulder and the State of Colorado. <br /> As such, the undersigned hereby grants to the agents, employees, and contractors of the Division of Reclamation, <br /> Mining and Safety, Department of Natural Resources, State of Colorado, permission to enter upon such lands to <br /> restore, reclaim, abate, control or prevent the adverse effects of past mining practices and to do all things necessary <br /> or expedient to protect the public health, safety or general welfare. Such activities are in the public interest. <br /> Consent is given to enter upon the above-described property for the length of time necessary to safeguard the <br /> following feature(s) in the manner described below. <br /> Feature ID Type of Feature Closure Method Legal Description <br /> SU26 Adit Grate Washburn No 2 M.S. # 11804 <br /> Inactive mine openings are inherently dangerous, unstable and subject to rapidly changing conditions. During the <br /> construction phase such changing conditions may make it necessary to safeguard the opening(s) using an <br /> appropriate alternative abatement method. <br /> The landowner has an existing responsibility to insure that any abandoned or inactive mine openings located on <br /> his/her properties are adequately safeguarded and all present and subsequent landowners are required to maintain <br /> the mine closure after the work has been completed as outlined in C.R.S. 34-24-110(1). This responsibility is <br /> unchanged by this consent to have these reclamation activities performed. <br /> Your signature must be witnessed by a notary public.
