The following items are incomplete for the <br /> application to be processed: <br /> 1. The original application signature page 9 by Premier Silica LLC as not received. <br /> 2. Application fee of $144.00 for a Construction Materials permit transfer was not received. The <br /> $115.00 submitted is$29.00 short. <br /> 3. Financial Warranty in the amount of$64,439.00. <br /> Per out telephone conversation today, you will be mailing in the remaining $29.00 application fee. The <br /> financial warranty is in process and will be forwarded to the Division upon completion. Please return the <br /> required documentation to the Division before September 10, 2017 to complete the application for <br /> approval. All completed permit documents are required to be submitted for consideration of your Request <br /> for Transfer of Mineral Permit and Succession of Operators Application. An incomplete application will be <br /> denied after the 60 day submittal request, if requested documents or information are not received.
