The status of the corrective actions are listed below. <br /> • COMPLIANCE PROBLEM#1: Approved storm water control and sedimentation prevention <br /> methods have not been implemented or have not been maintained. <br /> o On July 4, 2017, the Operator's submitted photographic documentation showing the <br /> installation of a silt fence along the western permit boundary and a berm along the <br /> eastern permit boundary. This problem has been resolved, however, the Division could <br /> not determine if the berm was constructed as approved and within the permit boundary, <br /> which may be a possible violation for affecting additional lands off-site. <br /> • COMPLIANCE PROBLEM #2: The mine identification sign needs to be refreshed or <br /> replaced, pursuant to Rule 3.1.12(1). <br /> o On July 3, 2017, the Operator's submitted photographic documentation showing a new <br /> mine identification sign was posted at the entrance to the site. After review of the <br /> submitted photo, the Operator will need to revise this sign before the compliance <br /> problem is resolved. "MLRD"will need to be changed to "MLRB", as this is an <br /> acronym of the Mined Land Reclamation Board.
