Compliance With Mine Plan NA - Support Facilities Not On-site <br /> N -Other N - Special Categories Of Mining <br /> NA -Processing Waste Y -Topsoil <br /> COMMENTS <br /> The Colorado Division of Reclamation,Mining and Safety(Division) conducted a complete inspection on June <br /> 22,2017 of the Peabody Sage Creek Mine; DRMS permit no. C-2009-087, operated by Peabody Sage Creek <br /> Mining,LLC. The Sage Creek Mine is currently in Temporary Cessation. The inspection was an OSM Oversight <br /> of a complete inspection. Tabetha Lynch and Dan Hernandez with the Colorado Division of Reclamation,Mining <br /> and Safety(Division)conducted the ground inspection. Dan Mackinnon with the Office of Surface Mining,was <br /> present for the entire inspectionh. Miranda Blomquist with TC accompanied the the inspection. The weather was <br /> mostly clear and hot. All areas of the mine were accessible.Maintenance items are bulleted below. <br /> AVAILABILITY OF RECORDS ❑Rule 5.02.4(1): <br /> The mine records were checked at the Foidel Creek Mine office. See the end of the report for details. <br /> HYDROLOGIC BALANCE-Rule 4.05 <br /> Drainage Control 4.05.1,4.05.2,4.05.3; Siltation Structures 4.05.5,4.05.6;Discharge Structures 4.05.7,4.05.10; <br /> Diversions 4.05.4; Effluent Limits 4.05.2; Ground Water Monitoring 4.05.13; Surface Water Monitoring 4.05.13; <br /> Drainage ❑Acid and Toxic Materials 4.05.8; Impoundments 4.05.6,4.05.9; Stream Buffer Zones 4.05.18: <br /> The upper sump, lower sump,pond 2,pond 3,pond 4, and Pecoco pond were observed and holding water. <br /> All discharge points are flowing, except NPDES#7,which is at a ditch near the office that was not discharging. <br /> Pump 42 was operating and dewatering. Pond maintenance items are listed below in general mine compliance <br /> section. <br /> GENERAL MINE PLAN COMPLIANCE: <br /> Conveyors salvaged from the Williams Fork mine C 1981044 are currently being stored just above the portal area. <br /> • Culvert on the West of Pond 2 needs the cattails cleaned out <br /> • Pond 3 needs maintenance at the outfall. <br /> • Vegetation needs to be cleared out between Sump 1 and 2 in the portal area <br /> • Portal area smart ditches need maintenance <br /> Number of Partial Inspection this Fiscal Year: 6 <br /> Number of Complete Inspections this Fiscal Year: 4 <br /> Page 2 of 7 <br />
