Mine Site Inspection <br />Mine Name: Southfield Mine <br />Permittee: Energy Fuels Coal, Inc. <br />Permit ID#: C-1981-014 <br />Inspection Date: May 15, 2017 10:15 —11:40 <br />Inspection Type: Partial Oversight Inspection <br />Weather Conditions: Warm, sunny / scattered clouds, dry <br />Participants: <br />Christine Belka, Office of Surface Mining Reclamation and Enforcement (OSMRE) #182 <br />Zach Trujillo, Colorado Division of Reclamation Mining and Safety (DRMS) <br />Southfield is a reclaimed underground coal mine located south of Florence, Colorado. Mining operations <br />are complete and the site is in permanent cessation. No operator representative was present during this <br />inspection. Mr. Trujillo indicated that the corporate entity responsible for this permit has recently been <br />dissolved and has not maintained right of entry to the permit area. This presents obvious regulatory <br />problems as the permittee is no longer viable and cannot enter the permit area to complete final <br />reclamation and bond release activities. <br />The mine identification signs posted at entrances from County Road 15 have been removed. Per Rule <br />4.02.2, identification signs are required to be posted at each entrance to the permit area from public roads. <br />The upper portion of the Magpie diversion channel is still devoid of rip rap. DRMS inspectors have noted <br />this issue and requested repair since last October. Per Rule 4.05.3, diversions must be constructed and <br />maintained to minimize adverse impacts to the hydrologic balance and prevent additional contributions of <br />suspended solids to off-site streamflow. The majority of the channel is well armored, although the corner <br />above the 9' culvert could be reinforced due to the severity of the angle. No erosion was noted and the <br />channel is currently stable. <br />The NW -MW road appears to be self -healing. A two -track road remains although vegetation is <br />establishing along the entire corridor. The North Well has been sealed and the pad reclaimed. The <br />equipment storage pad at the northeast end of the reclaimed road spur was hummocky and vegetation was <br />sparse, but grasses are establishing. Disturbed area markers are still in place in this area and no erosion <br />was noted. <br />The access road to the reclaimed substation pad is stable but overgrown with field bindweed (Convolvulus <br />arvensis), a List C noxious weed. Weed control and revegetation with desirable species is advised. <br />DRMS is citing three violations as a result of this inspection for 1) dissolution of Energy Fuels Coal, Inc., <br />2) failure to maintain right of entry, and 3) failure to maintain mine identification signs. <br />Page 11 <br />
