Prospective Successor Operator company name is not registered with the Colorado Secretary of <br /> State to do business in Colorado. Please register with the Colorado Secretary of State. <br /> 2. Application fee of$144.00 was not included with the application. <br /> 3. Performance Warranty operator name not registered with Colorado Secretary of State. <br /> 4. Demonstration of Legal Right to Enter not provided. See instructions enclosed. <br /> 5. Structure Agreements were not provided or a statement that structure agreements are not <br /> applicable. See instructions enclosed for Structure Agreements. <br /> Please return the required documentation to the Division before July 24, 2017 to complete the application <br /> for approval.
